A Compassionate Plan for Sickness, Aging, & End of Life

Who will be there when you need care?

As we age, live with serious illness, or approach the end of life, our need for practical and compassionate support often grows. A Circle of Care is a network of family, friends, neighbours, and community members who come together to support a person in ways that are meaningful, manageable, and coordinated.

Creating a Circle of Care means thinking ahead: Who are the people you trust? What kinds of support might you need? Who can realistically help, and in what ways? How will everyone communicate and share responsibilities without placing the burden of care on one person?

The Circles of Care model helps you identify your support network, clarify roles and responsibilities, establish ways to communicate, and consider the practical, emotional, social, and spiritual support that may be needed over time.

Grounded in dignity, connection, trust, and interdependence, Circles of Care offers an alternative to facing sickness, aging, or end of life in isolation. It recognizes that caring for one another is both deeply human and something we can intentionally prepare for.

Plan ahead. Gather your people. Create a circle of support that can grow and adapt as your needs change.
