As we age or face serious illness, the need for meaningful, compassionate support becomes more vital. The Circles of Care model invites us to rethink how care is given and received by centering it in relationships, trust, and shared responsibility.  This approach is grounded in values of dignity, connection, and interdependence. The vision is a world where no one faces sickness or aging alone. 

A “circle of care” is a support network of family, friends, and community members who help an individual with their care needs at home.  Planning for a care circle involves identifying potential support individuals, establishing a clear communication plan, and outlining the specific tasks or services each person can contribute.
